In this tutorial, we will be using WPForms for creating a questionnaire. 

WPForms is the most used and the best form plugin for WordPress. It helps you to create different types of forms using an easy drag and drop form builder. 

Firstly, install and activate the WPForms plugin on your WordPress. Once you have activated the plugin in your WordPress. 

Go to the settings page in your WordPress area and enter the license key. You will easily find the license key in your WPForms account on their website. 

Now visit the WPForms > Addons page and install polls addon and survey. You can the search bar and click the Install Addon button. 

Go to WPForms > Add New for creating a new form. 

We will use the survey form for your questionnaire. 

Once you have selected the template, it will open in the WPForms editor. 

For editing any field, simply click on it. You can see the editing view on the left-hand side of the screen. We made it optional unchecking the Required box. 

You will only see the How can we improve box if the user rates the experience. If you want it to appear on 3 or 4 stars, bring your mouse over the How can we improve box can click on the copy button. 

Now go ahead and click the OK button. 

You can edit the field seen on the screen’s left-hand side if you want.

Now set up the conditional logic for the field by clicking the Conditionals tab. Now set the numbers to 3 and 4 rather than 1 and 2. 

Now edit the how satisfied are you with tab. A Likert rating scale is 5 to 7 scale which is used for measuring attitudes and satisfaction. Click on the field to edit it. Change the labels of the columns and rows to the text you want it to appear. 

Make changes by clicking the save button.

How To Set Up Notifications For Your Questionnaire

To set up notifications, go to settings > notifications. Delete the {admin_email} available at the ‘Send To Email Address’ box and enter your desired email address. 

To add a questionnaire to your website, go to Pages > Add New. Click the +button  and select the WPForms block. 

Select your desired questionnaire form.

Now publish your post.
